STEP 1: Pre-Register Online

STEP 2: Download Your Consultant Agreement
The Required Tuition Agreement must be submitted after registering Online:

STEP 3: Fill Out the Agreement and Send in With Payment
Click, print and complete the form above and fax it to 480-907-1437 along with your deposit slip, copy of your Cashier's Check, or Western Union confirmation slip. Once FFPS Admin receives your completed tuition agreement your account will be activated. |
